Meridian AI
A governed AI workflow with human review at every consequential step
An internal copilot had shipped without evaluation, budgets, or a kill switch. Grafiesto rebuilt the surrounding system: typed outputs, a deterministic fallback path, an evaluation set that runs on every change, and an explicit human decision before any action that reaches a customer.

Inside the project
Context before craft. Decisions before decoration. The story behind the finished surface.
Not a reveal. A reason.
An internal copilot had shipped without evaluation, budgets, or a kill switch. Grafiesto rebuilt the surrounding system: typed outputs, a deterministic fallback path, an evaluation set that runs on every change, and an explicit human decision before any action that reaches a customer.
The starting point
What needed to change
The copilot worked well in demos and unpredictably in production. There was no evaluation set, so regressions were discovered by users. There was no cost ceiling, so a single malformed loop could run unbounded. And there was no gate between a model's suggestion and an action that touched a customer record.
The work
How we found the shape
Model output was reclassified as untrusted input: every response is schema-validated before it reaches application code, and validation failure routes to a deterministic fallback rather than a retry loop. Tool access was scoped per workflow. A frozen evaluation set runs in CI, so a prompt or model change that degrades a known case fails the build. Consequential actions were moved behind an explicit human confirmation with the model's reasoning shown alongside.
What shifted
The difference it made
The copilot ships changes on a normal release cadence instead of being frozen after each incident, and the team can point to a specific evaluation case when deciding whether a change is safe.
The best work starts with the honest version of the problem.

A useful first conversation needs no polished brief—just the real problem, the people it affects, and the change you want to see.
